Should my employer reimburse me for an annual copy of my driving record?

My employer wants all employees to sign a consent form that we need to annually submit a driving record at our own expense, can they force us to pay for it?

Nope. YOu can refuse. Odds are you are in a right to work state; meaning they can fire you for pretty much any reason; and they probably will. Lots of jobs have odd expenses that employees pick up out of pocket to keep their jobs

Ask them to split it with you.
